Debuting in September 2003, the third-generation Suzuki Wagon R (MH21S/MH22S series) adopted a clean, square-cut architectural styling with bold vertical headlights. Constructed on a revised chassis featuring Isolated Trailing Link (ITL) rear suspension, the MH21S maintained Suzuki's position as Japan's top-selling vehicle. In 2007, Suzuki launched the Wagon R Stingray variant, featuring a aggressive front fascia, translucent grille, and projector headlamps.

Engines were standardized around the 650 cc K6A 3-cylinder unit in naturally aspirated (54 hp) and turbocharged (60-64 hp) configurations. Suzuki also experimented with early mild-hybrid technology in the Wagon R Mild Hybrid version. Transmissions included 5-speed manuals, column- or floor-mounted 4-speed automatics, and optional 4WD.
