The first-generation Lexus GX 470 mid-size luxury SUV (chassis code J120) premiered at the North American International Auto Show in November 2002 for the 2003 model year. Constructed on Toyota's Land Cruiser Prado 120 body-on-frame architecture, it filled the slot between the RX crossover and LX flagship. Interior appointments included three rows of seating, bird's-eye maple trim, rear Adaptive Height Control (AHC) air suspension, and Adaptive Variable Suspension (AVS).

Power was supplied by a 4.7-liter 2UZ-FE V8 producing 235 horsepower, upgraded to 270 horsepower in 2005 with VVT-i technology, paired with a five-speed automatic transmission. Full-time 4WD incorporated a Torsen center differential. In 2004, Lexus introduced the Kinetic Dynamic Suspension System (KDSS) to improve off-road wheel articulation and reduce on-road body roll. A 2007 refresh added chrome door handles and updated alloy wheel designs.
